Update: No Yellow Forms Needed for Monday

From Mr. Kalish:

Dear Band Parents and Students,

I wanted to take a moment to apologize and fill you in on the last 24 hours of confusion regarding the Columbus Day Parade.

We have been doing this parade for many years and there was a miscommunication between the office and myself. I made the assumption that administration was aware of the parade and that it was on the school calendar because of our many years of participation. When an administrator heard about the trip and was unaware of it, they came down to let us know we needed the field trip forms filled out. I unfortunately was off campus at this time to take care of a serious family situation. I received a call from Mrs. Kovach that the administration was asking us to get the yellow forms in by Monday and if they weren’t in, then students couldn’t go on the trip. I then sent out the e-mail to cover our students with this directive from the administration.

Upon further review today with the front office, The yellow forms are for three reasons, to share with the teacher on the field trip an emergency medical contact for each student, to give permission for students to go on the trip, and to inform teachers the students will be missing.

We already have medical forms for each student, we also already have permission for students to participate band events through our initial band camp forms. The teacher awareness is taken care of through an e-mail to the staff from me. Attendance already has the names of the students missing. Therefore the yellow papers are, after all, not needed.

I apologize again for the confusion and the e-mail to scurry everyone to get the forms filled out. We now have clarity about how trips are taken care of and we are good to go.
